Forge BPV leaks air around the outside seal. Test yours
The outer seal groove is cut pretty deep. Only a small section of the o ring is exposed to seal to the housing. The problem is, a KO4 housing also has a groove cut into it therefore the o ring can't seal.
I did a boost leak test and saw that the Forge BPV won't seal. I talked to the company that I purchased it from, they were very helpful in this ordeal. Even sent me a new o ring, which acted the same as the last one. Still had a boost leak.
The stock BPV on the other-hand, because of how its designed doesn't suffer from this issue. Boost leak free.
Here are some pictures, mind you this is a new bpv, and tested with 2 o rings:
Here is a picture of a ko4 compressor housing, you can see the groove.

I had issues with the piston sticking after a year of use and I always kept it lightly lubed. I know they offer rebuild kits for them but I threw the sticker back in and never looked back as it's been trouble free. Should never have spent the money on it due to the stocker doing just fine.

Matt- for whatever reason at the bottom of the piston in mine over time it started to stick to the forge housing slightly, like it had a catch in it. It started to happen a little more frequently especially at lower throttle levels and would cause surging/heavy flutter from not opening. You could pull it out and it took some pressure to pop back free again. After that it would be fine for awhile. I could have got a new piston but after installing the stock one and not having issues, I just stayed with it.

My fuel trims have bounced around but never too bad. As in never setting a CEL. Ive seen a max of +7.8 on the stock tune but Id accredit that mostly to an aftermarket intake. On the tune, Ive seen no more than +/- 3.0 trims.
Its just when I switch back to the stock tune, I feel like even though the boost gauge is showing +1 to +3 psi under light acceleration, theres no *****. It may be just used to HPtune and forgot what stock feels like or theres a slight leak which would have a higher affect at low boost.
