High LTFT, need some advice

I've got an 06 SS/SC been driving it for almost 2 years, it doesn't have an aftermarket intake, and its running stock tune

The LTFT are staying around +17% to +19%, the short term is pretty normal from what I've seen staying slightly in the negative, and it's been like this since I got the car, throwing a random lean code (conditions for the last code below) So, the rear o2 seems ok on it's readings, I have a new throttle body (old one was acting up and throwing several codes, I tohught this was the lean code problem, but turns out it wasn't). Anyone have any thoughts or ideas what I could try or what it might be ? Also car runs great, pulls hard.

Closed loop
12% load
Short term = -6
Long term = 19
2875 RPM
39.5 Deg timing
48 Deg intake temp
Throttle position 27
MAF = 10.82

Did you check for vacuum leaks and check the front O2 sensor?

I cleaned the MAF today (no change), I've been through all my lines and shouldn't have any vacuum leaks, I will have a chance to check the o2 tomorrow, I will get some more info on it running down the street tomorrow as well and will post what I find up.

Reset the fuel trims and see if a code pops while it is trying to correct them.

If the LTFT improves with RPM, then I would lean toward a vacuum leak.

Ok, so an update on this, I went back and checked everything to make sure I didn't have a vacuum leak just to positively rule that out. I have noticed that every time that I look at the coditions of when the CEL was set it was while I was cruising on the interstate, usually something like 2800rpm 14% throttle. I had been leaning towards the o2 sensor but it seems within range and responsive 0-900mV with normal flux and graph spikes.
