Could use a little help

Sooo I have had this issue awhile and being searching for a answer....so far i have gotten no where. When coming to a stop (in and out of gear) my idle bounces and some time stalls out esp. when I am in gear. It doesnt usually stall if i am out of gear. I have had a few tunes on it seeing if the issue would be resolved but it hasnt yet. So that makes me believe its something mechanical. It usually gets worse the longer i drive the car. I figure maybe i have a vaccum leak somewhere??? If anyone could help me out I would much appreciate it.

Mods Include...2.7" w/ 60lbers, header/DP/catback, intake, front mount he

mine would kinda do the same thing, and if i gave it over 50% throttle it would go into limp mode. turned out that the throttle body was bad and would lock up anything over 50% throttle. replaced the TB and all probs went away.
