Exclamation Jerking/No Acceleration!

Hey guys and gals! We need some desperate help!

My wife's cobalt (2.2 5 speed LT manual) has been having an ongoing problem of it "jerking" randomly going down the road.

Today that just got MUCH worse.

We were driving when all of a sudden we crossed some railroad tracks nice and softly then when to shift into second @ about 15 mph and the car wouldn't accelerate, all of a sudden about 3 seconds later it caught up and took off like crazy!

Need some input/help to figure out what it could be!
