I have a used w580i it does have some problems; i bought it off ebay refurb and it would never take pictures unless you did the panoramic photo mode, the buttons on the front are wearing. comes with:
Data Cable
512mb card
stereo headset
manuals etc.
Runs on gsm carriers (sim card) not sure if it needs to be unlocked like i said bought it of ebay and was already unlocked havent tried putting a diff sim card in it. It should work with any carrier (GSM obviously)
