I replaced the clutch, pressure plate, flywheel, and throwout bearing about 10000 miles ago when i put the new engine in. I got it all back together with a new clutch disk and i went ahead and replaced the TOB as well and all is working well agian.

So i took the transmission out and went ahead and pulled the pressure plate and found the clutch disk like this. Bunch of metal chipped off around all the springs and one of those little hex key uts was completely out and rattling around in there.
