(MD) Passing emissions without cat
I'm going to replace my broken dp with a ZZP one that has a gutted cat. I know I'll be able to pass visual inspection, but if I had got an angled O2 sensor adapter for the 2nd sensor so I don't throw a code, will I have any potential issues with passing inspection?
Not once have i had them hook up a sniffer to my exhaust, it's always been through the OBDII port.

I don't know how Maryland is, but I passed emissions last year when my car was catless.
My GMR S3 PCM tunes out the rear O2 sensor, and as long as I have no codes present, I pass.
PA only checks the gas cap & looks for emission codes. Thankfully.
To answer your question, an angled defouler should prevent you from throwing a code.

It would read passed if defeated in hpt. My car does pass emissions but I've flashed it too soon before I needed to be inspected and couldnt get the drive cycle done so I just flashed a tune with all the codes tuned off and it reads passed the instant the car is started the first time.
hptuner deletes the test itself, unless you have someone accually looking for the specific cat test, you will be fine. mine has been deleted many years, and many emissions tests, and never had an issue.
