Several Engine Problems

So I've been noticing recently that the problems with my car are becoming more frequent and worse. I'll go through a list of everything:

1. Rattle noise at certain RPM and certain engine conditions. Sounds like metal on metal, possibly cat.

2. P0068 & P0069 at same time causing limp mode. Happened while cruising at 60, very little throttle.

3. P0069 by itself, random after starting car, cleared with scanner and hasn't come back.

4. Engine dies while idling at stop today. Has a tough time starting, doesn't sound like battery. Starts with no problem after 6 seconds of intermittent cranking.

5. Rough idling at times, e.g. cold start, after #4 above, randomly too.

Anyone have an idea what these things can mean? Do they seem related? They seem like signs of an impending doom for my car. I replaced the plugs 8,000 miles ago, and I'm thinking of pulling them to check their condition. Maybe I gapped then way off, although I am confident I did them accurately and to the proper spec.

After #2 happened I cleaned a bunch of the intake air sensors with MAF cleaner. I cleaned the MAF, throttle body, and the sensor right next to the supercharger inlet (SCIP).

What else should I be checking/troubleshooting? This stuff is making me crazy!

Could be the belt tensioner, mine makes noise like crazy when slowly shifting from 1st to 2nd. Or when I just rev it up in neutral.

I will check that. Anyone know how I can test the tensioner?
