Sweepstakes Entry Locations and Dates: You may enter the sweepstakes at participating Chevrolet dealers in the Southeast Region (includes Atlanta, GA; Charlotte, NC; Norfolk, VA; Raleigh, NC; West Palm, FL; Jacksonville, FL; and Knoxville, TN) from September 27, 2004 through November 13, 2004, while supplies of entry forms last (the "Promotion Period"). You may also enter online through November 13, 2004. See entry details below.
Who is Eligible: To enter, you must be a legal resident of the 50 United States or District of Columbia ("D.C."), age 18 or older as of date of entry, and must have a valid driver's license. The following classes of persons are NOT eligible to enter:
Employees, officers, and directors of participating Chevrolet dealers, General Motors, Chevrolet Motor Division, American Hole 'n One, each of their respective affiliated companies, subsidiaries, distributors, dealers, retailers, printers, advertising, and promotion agencies, and any and all other companies associated with this sweepstakes, and the members of the immediate families or households of any of the above.
How to Enter: To enter online: Beginning at 8:00 am, Eastern Standard Time, on September 27, 2004 visit www.thirdday.com, click on the appropriate banner, and follow the on-screen instructions to submit your entry. Online entries must include your name, address, age and day and evening phone numbers, along with all other required information. Entries will be deemed made by the authorized account holder of the e-mail address submitted at the time of entry. The "authorized account holder" is defined as the natural person who is assigned to an email address by an Internet access provider, on-line service provider, or other organization responsible for assigning email addresses for the domain associated with the submitted e-mail address. All on-line entries must be transmitted and received by 11:59 pm , Eastern Standard Time, on November 13, 2004.
To enter at a dealer location: Visit a participating Chevrolet dealership in one of the cities designated above during regular business hours during the Promotion Period and complete an official entry form (while supplies last). You must include your name, address, age and day and evening phone numbers on the official entry form, along with any other required information. Sign and date the entry form, and deposit it into the official registration box at the display table. Last time to enter will be close of business on November 13, 2004 or when official entry forms have been depleted, whichever occurs first. No mechanically reproduced or facsimile entries permitted. All entries must be hand-printed. Limit one entry per person or household for the entire Promotion Period, regardless of method of entry.
Sponsor is not responsible for lost, late, misdirected, damaged, incomplete, or illegible entries, or for entries submitted in a manner not specified in these rules, all of which will be disqualified. The Sponsor is further not responsible for any of the following: (a) computer system, phone line, hardware, software or program malfunctions, or other errors, failures or delays in computer transmissions or network connections that are human or technical in nature; (b) incorrect or inaccurate entry information whether caused by Internet users or by any of the equipment or programming associated with or utilized in the Sweepstakes or by any technical or human error, which may occur in the transmission, receipt or processing of entries; (c) damage to an entrant's or any other person's computer related to or resulting from accessing or downloading any materials in this promotion.
Sponsor reserves the right, in its sole discretion, to cancel or suspend the online portion of this Sweepstakes should virus, bugs or other causes beyond the control of the Sponsor corrupt the administration, security or proper play of the Sweepstakes and in such case to randomly award the prize from among eligible online entries received as of date of cancellation/suspension combined with entries received at all dealer locations through the scheduled end date, in accordance with these rules.
CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS. Winner Selection & Notification: A random drawing will be held on or about 11/29/04 from among all eligible entries received from both entry methods combined. One (1) winner will be selected to receive the grand prize described below. Winner will be notified by phone and/or mail by or about 11/30/04. If winner cannot be reached, or prize notification is returned as undeliverable, prize will be forfeited and awarded to an alternate winner.
Prize Award & Delivery: The grand prize is a 2005 Chevrolet Cobalt with an Approximate Retail Value not to exceed $21,995 (color and options to be determined solely by Sponsor). The grand prize winner must take delivery of vehicle from the designated Chevrolet dealership nearest winner's home, or as otherwise instructed by sponsor. All other costs related to prize acceptance and use are the responsibility of winner, including but not limited to income taxes, title, insurance, license and registration fees.
Prize is non-transferable and no cash redemption or prize substitution allowed, except at sponsor's sole discretion or as provided herein. Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable. All of the following will apply to the Grand Prize winner:
1)Winner will be required to complete and return, within a specified time period, a W-9 form, and an affidavit of eligibility and liability/publicity release (where permitted);
2) Winner will be required to present a copy of a valid social security card, a valid driver's license and proof of insurance as a condition of receiving prize;
3) Winner will be issued an IRS Form 1099 reflecting the value of the prizes;
4) Winner will be responsible for paying all income taxes on prize.
Failure to collect or properly claim prize in accordance with these rules will result in forfeiture of the prize. If forfeited for any reason, the prize will be awarded to an alternate winner. Total maximum prize value for this promotion: $21,995.
