You have to replace the entire seatbelt assembly. I bought a new one from CED, and I think it cost me a little over $100. It includes the seatbelt, retractor, guides, ...the whole works.
To replace it, you need to remove all the interior trim in the passenger rear of the car. I had to use an impact wrench to break the seatbelt bolts loose. The rest of the procedure is pretty easy. If something is in the way, remove it. Take the old one out, and put the new one in.
