5th Gear Grind

So just yesterday my 05 LSJ starting grinding when shifting into 5th gear. It's the only gear that does it. I did some research on synchros but I still have a couple of questions for all the experts on the forum here.

1. What is the typical solution for you guys dealing with synchros that have worn out? New transmission? Rebuild?

2. Can I get away with and should I just rev match and shift without the clutch to eliminate grinding? Or is that a recipe for disaster?
