06 SS Antenna help

The antenna bracket on my car broke. What do I need to replace it, part numbers? I cant tell how the base was even attached to the bracket. Also, is the piece that the antenna screws onto called "antenna ground plate"?

It seems like the original owner put loctite on the shaft or something because his antenna got stolen before. I crushed the base metal with vice grips and still cant get the mast off.

I ended up having to use vice grips too and it's now held on by a zip tie.
