Bad clutch disc?

Bad clutch disc?
OK I've been trying to pinpoint the problem of my Cobalt. I think it's the clutch disc but Ijust want to be sure before I shell out the money.
1. Clutch is very slow to engage. Pedal will be completely depressed and clutch is slipping.
2. Clutch won't engage until in the higher RPMS. And once it engages it's fine until I take my foot off the gas. Then it starts all over again.
3. Have no problems getting the car into gear at all
4. No boost pressure (Idk if this is relevant but thought I would add it in)
5. No vibrations at all coming from car.
6. The car runs fine besides this.
Like I said I think it's the disc but I want to be sure. What do you guys think?
